Schedule a Telehealth E-Visit<\/h2>\n\n\n\n

Howard Head Sports Medicine provides Telehealth Services in addition to in-person appointments. \u00a0A Telehealth E-Visit is a scheduled one on one appointment with a licensed Physical or Occupational Therapist. Due to Colorado State Law, Telehealth can only be provided to those who are physically located in the State of Colorado. Please contact your provider directly if you do not meet this criterion.\u00a0<\/p>\n\n\n\n

  1. To schedule your Telehealth E-visits, please call (970) 476-1225<\/a>\u00a0<\/strong>and request a telehealth appointment, a patient representative will schedule your E-visit appointment.\u00a0
    \u00a0<\/li>\n\n\n\n
  2. Patients will receive an email confirmation with a link to the E-visit.  A few minutes before your scheduled appointment, click on the link sent to you, and you will be connected via ZOOM and\/or Medbridge, both are secure and HIPAA compliant platforms.

  3. After your visit is scheduled, your provider may send a health history form, consents, and other applicable forms that will need to be completed and returned prior to the start of your appointment.

  4. You will need a strong internet connection, along with audio and video capabilities. Please be in a quiet, well-lit location to provide the best experience.
     <\/li>\n\n\n\n
  5. During your E-Visit appointment you will have the opportunity to discuss your condition\/injury with your therapist. Skilled instruction, education, home exercise programming, and professional recommendations can be provided.<\/li>\n<\/ol>\n\n\n\n

    Telehealth visits are available for your healthcare needs. Telehealth visits are billed to patients\u2019 insurance like any other office visit and are typically covered by insurers, including Medicare and Medicaid. Co-pays, deductibles, and visit limits may apply, patients should consult their specific insurance plan. If you prefer, our self-pay rate is $95 per session. <\/p>\n\n\n\n

    In-person visits are always available at any of our\u00a0Eagle County, Summit County, and Roaring Fork Valley locations<\/a>.<\/p>\n\n\n\n
